Maybe his IP that was permabanned was one of the proxy ones, and the original ban for his original account has ran out?

Bans don't expire unless you specify the number of days for the ban.

Something must have changed with his net service, something not apparant. That's gotta be it. Many ISPs use dynamic IP addresses for their customers. So an IP ban will only work as long his current connection to the Internet lasts.
